Ways you can get a Form

Submit the Completed Form to CGMA-HQ

Preferred: Complete and submit the Retired Allotment Form or CGMA-45 Retired Allotment Authorization (pdf) online

If you do not want to complete the form online, you may simply print the form, complete it manually and submit to CGMA-HQ via one of the following methods:

  • Scan or take a picture of the form and email it to CGMA-HQ at donations@cgmahq.org
  • Fax the form to: (703) 875-0344
  • Mail the form to: Coast Guard Mutual Assistance, 1005 N. Glebe Road, Suite 220, Arlington, VA 22201
