Donations may be made "in honor" of another individual.

A donation in honor of another is an excellent tribute to service and dedication of a individual who has been a special part of your life while supporting the men and women of the U.S. Coast Guard.

If you complete and attach an Honorary Donation form, with your donation, CGMA-HQ will provide written acknowledgment to you and, if you desire, to the individual or their family you are honoring.

are deductible for income and estate tax purposes

Submit the Completed Form to CGMA-HQ

You may complete the Honorary Donation Form CGMA-48 (pdf) online, when completed, the form may be printed. Or, If you do not want to complete the form online, you may simply print the form and complete it manually

Honorary Donations (checks or money orders made payable to Coast Guard Mutual Assistance or CGMA) along with the Honorary Donation Form - if completed - should be mailed to:
